Definition of Service:

Substance Abuse Case Management Services are designed to support adults diagnosed with Substance Abuse Disorders, including those with co-occurring mental health conditions. The goal is to help individuals access necessary resources, achieve recovery goals, and reintegrate into the community successfully. Services focus on needs assessment, care coordination, advocacy, and transition planning.

Responsibilities:

The Substance Abuse Case Manager will:

  • Conduct comprehensive assessments to identify individual needs.
  • Develop and implement Individual Service Plans (ISPs) using accepted placement criteria within 30 days of service initiation.
  • Coordinate care and services across providers, agencies, and systems to ensure seamless support.
  • Link individuals to community resources that align with their recovery and personal goals.
  • Provide education, counseling, and advocacy to support adherence to the ISP and promote self-sufficiency.
  • Monitor service quality through regular contacts, site visits, and home visits.
  • Collaborate with significant others (with proper authorization) to enhance community adjustment.
  • Assist in planning for life transitions, ensuring continuity of care.
  • Track and document individuals’ progress and adjust ISPs as needed.
  • Maintain accurate, timely records and comply with all agency policies and regulatory standards.
  • Participate in team meetings and contribute to the continuous improvement of case management services.

Requirements and Qualifications:

Education and Experience:

Candidates must meet one of the following combinations of education and experience:

  1. A bachelor’s degree in social work, psychology, psychiatric rehabilitation, sociology, counseling, vocational rehabilitation, or human services counseling and:
    • At least one year of direct substance abuse-related experience providing services to individuals diagnosed with Substance Abuse Disorder; or
    • At least one year of clinical experience working with individuals with co-occurring Substance Abuse Disorder and mental illness.
  1. Licensure in the Commonwealth of Virginia as a Registered Nurse (RN) and:
    • At least one year of direct substance abuse-related experience providing services to individuals diagnosed with Substance Abuse Disorder; or
    • At least one year of clinical experience working with individuals with co-occurring Substance Abuse Disorder and mental illness.
  1. Certification as a Board of Counseling Certified Substance Abuse Counselor (CSAC) or CSAC-Assistant under supervision as defined by regulatory standards (18VAC115-30-10 et seq.).

  • Strong organizational and problem-solving skills to manage multiple priorities effectively.
  • Proficiency in verbal and written communication with both internal teams and external stakeholders.
  • High level of professionalism and exemplary customer service skills.
  • Competency in using electronic health record systems and basic office software.

  • Empathy and active listening skills to understand and support client needs.
  • Ability to work collaboratively within a multidisciplinary team.
  • Cultural sensitivity and awareness in working with diverse populations.
  • Flexibility and resilience in adapting to changing circumstances and challenges.

Other Requirements:

  • Valid driver’s license and access to reliable transportation for community and home visits.
  • Background check clearance in compliance with agency policies.
